Car owners with a television are 27 percent more likely to suffer heart attacks than people who have neither, according to a global study published last week. The study – covering more than 29,000 people in 52 countries – showed that working up a light sweat may be the best preventative medicine against heart failure.

“This study shows that mild to moderate physical activity at work, and any level during leisure, reduces the risk of heart attacks,” said lead researcher Claes Held, a professor at Uppsala University in Sweden.

It also “extends previous findings of the protective effect of leisure-time physical activity to low- and middle- income countries.”

Held and colleagues poured over data collected from 1999 to 2003 for the so- called Interheart study. They compared one group of more than 10,000 middle- aged men and women who had had a single heart attack with an even larger cohort with no history of cardiovascular disease.

Physical activity was divided into four levels of exertion: completely sedentary at one extreme, and, at the other, doing hard physical labor on the job or heart-pounding aerobic exercise while at play.

Not surprisingly, the study, published in the European Heart Journal, found that exercise is good for the heart. But the effectiveness of physical activity varied depending on the setting and intensity.

Any kind of workout during leisure time was seen as a plus, with heart attack risk dropping 13 percent for mild activity and 24 percent for moderate or strenuous exercise.

The advantages were similar for light and moderate exercise on the job. Unexpectedly, heavy physical labor did not reduce risk at all.

The team also investigated whether owning an automobile, motorcycle, stereo, TV, computer, land or livestock influenced heath outcomes.

“Subjects who owned a car and a TV” – 25 percent of the respondents in poorer and middle-income nations, and two-thirds in rich ones – “were at higher risk of myocardial infarction,” the researchers concluded.

Diabetes and high blood pressure was also more common, but only in the developing world. What accounted for the link?

Another set of figures from the study points to a common-sense explanation. Possessing these consumer items made it about four times more likely in poorer and middle-income countries – and twice as likely in wealthy ones – that people would be sedentary, especially at work. Basically, TVs breed couch potatoes, and vehicles discourage walking.

Emiline Van Craenenbroeck and Viviane Conraads of Antwerp University Hospital Belgium, noted in the same journal: “Staying physically fit throughout life may be well be one of the easiest, cheapest and most effective ways to avoid the coronary care unit.”

Vitamin D supplementation and exercise may help prevent older men and women from falling, suggests a new review of 54 studies by the U.S. Preventative Services Task Force (USPSTF).

Falls are the leading cause of death, disability and loss of independence for people 65 years and older. About one in every three adults in this age group who are not institutionalized will fall in a given year. More than $20 billion in annual health care costs may be associated with those events.

“The number of falls among older adults is increasing significantly every year, and it's not because the percentage who fall is increasing. Rather, it's because the number of older adults is growing and will continue to grow,” lead researcher Dr. Yvonne Michael of Drexel University in Philadelphia, told Reuters Health.

“Understanding how to prevent falls is crucial,” added Michael, who started the review while at Kaiser Permanente Center for Health Research, in Portland, Oregon.

Michael and her colleagues set out to determine what interventions might be useful for primary care physicians to prescribe to their older patients. They identified 54 relevant studies, which included a total of more than 26,000 participants.

Getting exercise and taking vitamin D supplements appeared to offer the most potent prevention among the interventions investigated, which also included vision correction and modifying the home environment, report the researchers in Annals of Internal Medicine.

No risks of other physical harms were found to be associated with any of the strategies.

Exercise, on average, cut the seniors' risk of falls by 13 per cent. Specific interventions ranged from programs that improve balance, strength and flexibility to weight training, general physical activity such as walking or aerobics.

Vitamin D supplementation — with or without calcium — reduced the risk of falling by an average of 17 per cent. Half of the trials used daily doses above 800 international units (IU).

A previous review had found vitamin D to be ineffective in preventing falls. However, Michael pointed out, one of the large studies included in that analysis was determined not to be of high enough quality to be incorporated into the new review. In its place, the researchers included four other studies they deemed more appropriate.

“We need to do more study,” Michael said. “But, on the basis of this review, I think there is fairly stable evidence that physicians can successfully reduce the per cent of (those who fall) through vitamin D supplementation.”

The researchers acknowledge some limitations to their review, including the issues inherent in any effort to combine results from multiple studies, such as a lack of consistency in terminology, interventions and measurements.

The current findings will serve as the basis for recommendations from the USPSTF, an independent panel of experts in prevention and evidence-based medicine. A draft, expected soon, will be available for public comment before becoming finalized.

In an accompanying article in the same issue of the Annals, researchers addressed the unique challenges in formulating any prevention-related recommendations for older adults.

“As we age, prevention is no longer a 'one size fits all' endeavor,” lead author Dr. Rosanne Leipzig of the Mount Sinai School of Medicine, in New York City, told Reuters Health in an e-mail.

Older men and women are often burdened by an array of complicating conditions and other issues.

“We can't point to just one thing that is the cause of a fall in an older adult. A number of factors can come together to lead to a fall,” added Michael. “Still, there are still some things that physicians are able to recommend or prescribe to their patients.”

“Hopefully this will make it easier for physicians to make treatment plans that include exercise and vitamin D, and will be covered by insurance,” she said.

Advocates say while numbers are rising nationwide and locally, that means they’re reaching more victims; but not enough. The goal now is to have every physician screen for domestic violence, to get that help to get more people. “Even when people do call and contact us, that is a small percentage of the reality of the violence that’s being perpetrated in our community,” said Wapoint Co-Director of Domestic Violence & Sexual Assault Services Michael Shaw.

It’s a sad reality in Eastern Iowa, as Waypoint saw a 30% increase in call volume to their domestic violence crisis line last year.

“Ask every woman, every time in a private space,” said Shaw. It’s a simple request from Waypoint’s Michael Shaw; not for his staff, but for physicians. “In the ideal world it would be any physician that is interacting with a woman that’s doing that universal screening just to ask that question.”

Recommendations from two national groups hope more doctors will do just that. Saint Luke’s Domestic Violence Task Team trains physicians on what to look for when they’re meeting with a patient. “Sometimes it’s physical signs, other times it’s their behavior,” said Saint Luke’s Hospital Social Worker Sarah Hefflefinger.

In many hospitals restrooms and exam rooms, you might see posters about domestic violence, but advocates say it’s not enough. Victims need one-on-one conversations to ensure they’ll find resources. “We have the nurse and the physician see the patients and ask how safe they feel at home or if they have any concerns of their well being in their current relationships,” said Hefflefinger.

“The likelihood is that more women will say this is what’s happening to me and have that language to talk about it and we’ll connect them with an advocate,” said Shaw.

Wednesday, the Iowa Senate Judiciary Committee begins discussion on the Violence Against Women Act that was last revised in the early 1990s. Advocacy groups hope lawmakers will change policy, and give them more funding to help the growing number of victims.
